Jo Dee Messina's "Gear Grab & Greatest Hits Giveaway"

Hi-Fi Fusion will provide:

*All station contestants will be entered into a collective Grand Prize drawing for a DVD/VCR Player, TV, and Video Camera plus an autographed DVD copy of Jo Dee Messina's Video Collection and a DVD copy of Mirimax's "View From the Top" featuring Gwyneth Paltrow
*Three autographed DVD prize packs, including an autographed DVD copy of Jo Dee Messina's Video Collection and a DVD copy of Mirimax's "View From The Top" featuring Gwyneth Paltrow to be mailed directly to the three winners from the station’s listening audience
*Graphic art (aka banners) for the "Gear Grab & Greatest Hits Giveaway" to place on the homepage and contest page of the station’s website
*A "Gear Grab & Greatest Hits Giveaway" web page branded expressly for the participating station allowing listeners to enter the contest and enjoy additional content such as an e-card, song streams, wallpaper, screensaver, etc.
*A copy of all names, e-mail addresses, etc. collected in each station’s market from their respective "Gear Grab & Greatest Hits Giveaway"

The participating station will provide:

*A promotional commitment to this online event for two weeks running up until the announcement of the station’s contest winners (please see affidavit below)
*A minimum of 25 mentions per week (6am – 6pm) during the contest period [Note: the mentions can be live promos or recorded spots as desired.]
*Jo Dee Messina consideration in each mention (for example, “Check out www.WXYZ.com to enter to win a DVD/VCR Player, TV, and Video Camera in Jo Dee Messina's "Gear Grab & Greatest Hits Giveaway" – courtesy of WXYZ.”)
*A station logo for placement on the "Gear Grab & Greatest Hits Giveaway" page
*Placement of at least one "Gear Grab & Greatest Hits Giveaway" banner (supplied by Hi-Fi Fusion) on the homepage of the station (for the entire two week run of the contest)

On-Air "Gear Grab & Greatest Hits Giveaway" Promotion - Certificate of Performance

I hereby acknowledge and attest that the station stated below will participate in Hi-Fi Fusion’s "Gear Grab & Greatest Hits Giveaway" promotion. This station agrees to air a total of 50 pre-recorded spots and/or live mentions, as described above, promoting this Internet promotion located on the station’s website during the two week period designated below per this agreement.

My signature in this document may be used to provide proof of participation in the promotion and shall represent an accurate and true Certificate of Performance. I also understand that this becomes an official document and may also be reviewed by the F.C.C. Falsely certifying that commercials were broadcast may constitute a violation of Paragraph 78:1205 of the F.C.C. Rules and Regulations.
